(Single Employee, Employee and Children, Employee and Spouse, or Family). 

c  Copy of current plan  design.

c  Current health care carrier.

c  Current Pharmacy Benefit Manager (if applicable).

c  Current rates and factors.

c  Current contract (12/12, 15/12, Paid, etc.).

c  Benefits included in current aggregate (medical, dental, prescription, vision).

c  3 years of claims experience, if available, by month
 (current year plus two previous years).

c  Any claims with diagnoses and prognosis.

c  Renewal date.

c  Effective date requested.
